This plain-language guide by the American Civil Liberties Union aims to explain SDM to prospective decision-makers and to provide them the tools to create SDM agreements on their own. It is divided into eight chapters that 1) describe who and what is involve in SDM, 2) prompt decision-makers to reflect on the choices they make every day, 3) give examples of what SDM may look like in practice, 4) provide a worksheet to help decision-makers identify which decisions they make alone and which they make with the support of others, 5) provide a checklist for thinking about who would be good supporters, 6) give tips on how to approach prospective supporters, 7) describe how to fill out an SDMA, and 8) provide additional resources to learn more about SDM. In its appendix, the guide includes a model SDMA form, plain-language HIPAA Authorization, Authorization to Disclose Educational Information, and FAQs for supporters.
